Role Overview
- Schedule: 3-4 days a week within Monday to Friday
- Provide patient care in a skilled nursing facility by conducting rounds in their short-term units
- Conduct documentation at home with provided iPad laptop within 48 hours of seeing patients
Key Responsibilities
- Perform assessments and therapy sessions
- Collaborate with facility staff to enhance patient care
- Ensure timely and accurate documentation
Requirements
- Hold a valid full license as either
- LPC licensed professional counselor
- LMHC licensed mental health counselor
- LCSW Licensed Clinical Social Worker
- Experience in healthcare or long-term care is preferred but not required
Expected compensation is estimated salary range of $44,000 to $64,000 with no cap on productivity income potential
