As a post-acute care medical director and primary care physician you will be a part of an integrated care team providing primary care services in post-acute care facilities including skilled nursing and assisted living. Our patient population includes long-term care, short-term rehab, memory care, and clinically complex patients and residents.
As a TeamHealth post-acute medical director, you will provide oversight, guidance, and mentorship to our clinicians within your facilities. In this role, you will conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care, represent TeamHealth on appropriate external levels identifying, engaging and establishing/maintaining relationships within the healthcare community, and collaborate with clinicians, hospital leaders and others regarding multidisciplinary care and services for patients. Other responsibilities will be to collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues and perform other duties and responsibilities as required, assigned, or requested.

TeamHealth will help you develop a collaborative team approach between the facilities clinical team, supervising physician in the market, and additional service providers in wound care, behavioral health, or infectious disease specialist both locally and nationally. If you have a background in providing primary care in internal medicine, family medicine, emergency medicine, urgent care, physiatry, PM&R or geriatrics this could be a great fit for you.
The typical schedule is Monday through Friday with flexible daily hours. We have an overnight call service provided 7pm to 7am by our APC call team. Clinicians rotate backup call to the call team for occasional escalation needs in the local market and weekend day call.
Our compensation is a productivity based model with a base salary for full-time clinicians that offers autonomy and control to maximize your income potential.
