This role performs and coordinates work with the patient, family or support members, inpatient case management team, and interdisciplinary care team members during post-acute admission for appropriate utilization of services, length of stay and safe discharge plan. As the nurse practitioner you will communicate regularly on post-acute care of specific patients, as well as population level care and performance. You are responsible for providing quality care services for the patients, diagnosing them, and administering medications, as well as working with patients with a short-term illness or those patients recovered from medical conditions that require ongoing treatment and medications. Candidates should be confident in their ability to monitor patients' medical histories, scheduling follow-up checkups, as well as other procedures; manage patient caseload by performing evaluations and implementing therapeutic interventions through ADL training and exercise. We are seeking candidates with excellent communication and organizational skills.
