United High Deductible Plan

The United High Deductible Plan has a $2,000 individual and $4,000 family in-network deductible. This deductible is combined to cover prescriptions and well as normal medical costs. Further, for coverage greater than individual, $4,000 of expense must be met by the family before the plan will pay.

100% of "preventive" services will be paid by the plan without the deductible being met.

For procedures other than preventive services, 100% of out-of pocket costs are paid by the plan after the deductible is met.
