Tobacco Premium

Employees or dependents on TeamHealth health insurance who use tobacco will pay $50 more per month for health insurance than non-tobacco users. Employees can validate that they are non-tobacco users so that the tobacco premium will not be assessed. Tobacco users who stop for six months may then qualify for removal of the tobacco premium.
