Caremark Prescription Coverage

This plan is a co-insurance plan administered through Caremark. When comparing to Medicare D, the coverage is considered "creditable" for all plans except the High Deductible Plan. In that case, the coverage is not creditable (see "forms" for complete notification). In general, co-insurance is 35% of the cost of the drug at retail and 25% of the cost of the drug at mail, for a three month supply. Penalties apply when generics are not purchased when available, or when non-preferred brand drugs are purchased instead of preferred brand drugs. As a part of the TeamHealth Wellness Program, effective October 1, 2005, Asthma, Cardiac, and Diabetic class prescriptions can be purchased at mail with a 20% coinsurance vs the 25% coinsurance for other mail order prescriptions
